Terms & Conditions:
The referred patient must start treatment for you to qualify for the gift card.
The referred patient must be submitted through this referral form prior to their first appointment to qualify for the gift card.
There is no limit to the number of friends you can refer!
We will contact you after your friend begins treatment to coordinate gift card pickup.
This offer is valid for new patient referrals only and excludes immediate family members (e.g., referring a sibling or child does not qualify).
